Assured Coordination

Privacy

Your information is personal. Here is how I look after it, in plain English.

Assured Coordination is run by me, Steve Jones, as a sole trader. I handle personal, disability and health information in line with the Privacy Act 1988 and the Australian Privacy Principles.

What I collect and why

To provide support coordination, I collect things like your name and contact details, your NDIS number and plan details, information about your disability, health and support needs, your goals, and the contact details of people and providers involved in your supports.

I only collect what I need to coordinate your supports, and I use it only for that purpose.

The contact form on this website collects your name, phone, email, plan type and message. I use these only to reply to your enquiry.

How I store it

Your information is kept secure, using password-protected systems and devices. Only I can access it, and I keep it only for as long as I need to or as the law requires.

Who I share it with

I only share your information with your consent, for example with your providers, your plan manager or the NDIA, so your supports can work.

The only exception is where the law requires or allows me to share it, such as to keep someone safe.

Seeing and correcting your information

You can ask to see the information I hold about you at any time, and ask me to correct anything that is wrong or out of date. Just get in touch.
